hospital readmission
The standards of a great hospital. The feeling of family.
We started this agency after watching our own parents navigate care that was rushed, rotating and impersonal. So we built the agency we wished existed:
- Nurse-led, always. An RN writes every care plan, supervises every caregiver and re-evaluates at least every 60 days, or the moment something changes.
- The same faces, every visit. One primary caregiver, one familiar backup. We measure caregiver consistency monthly and publish it to families.
- Updates after every visit. Family members get a plain-English note through our family portal, wherever in the country they live.
- Employees, never contractors. Screened, trained, bonded, insured and proud to wear our badge.
- Straight answers about money. We verify Medicare, Medicaid, VA and private benefits before care starts and quote private rates in writing.

Trusted by your neighbors
Real families, from Summerlin to Boulder City.
“After Dad’s stroke we were lost. Their nurse came to the hospital before discharge, walked us through every medication, and had a plan on the fridge by day two. Six months later he is walking to the mailbox again.”

“I live in Ohio and manage Mom’s care from 2,000 miles away. I get a note after every single visit. When her blood pressure spiked, they caught it, called her doctor, and called me, all before lunch. That is worth everything.”

“I told my daughter I would rather stay in my own kitchen than move to any facility, no matter how nice the brochure. My aide Maria has been with me three years now. She is family.”

Refer with confidence. We report back, every time.
Hospitals, primary care physicians and specialists across Clark County refer to us because we treat communication as part of the treatment: same-day referral response, visit documentation back to your office, and a nurse you can reach directly, not a voicemail tree.
- Referrals accepted by phone, fax or secure portal, 7 days a week
- Patients seen within 24 hours of discharge, weekends included
- OASIS-compliant documentation and outcome reporting

What is the difference between home health care and in-home care?
Home health care is clinical care, such as skilled nursing, wound care and physical therapy, ordered by a physician and often covered by Medicare. In-home care, sometimes called home care or personal care, helps with daily living: bathing, dressing, meals, medication reminders and companionship. Many of our Las Vegas families use both together, and we coordinate them under one care plan so nothing falls through the cracks.
How quickly can care start?
For most families in the Las Vegas Valley we can complete a free in-home assessment within 24 hours of your call and begin care the same week, often the next day. For hospital discharges we routinely have a caregiver or nurse at the home on the day your loved one arrives.
Does Medicare pay for home health care in Las Vegas?
Yes. If a physician certifies that skilled care is medically necessary and your loved one is homebound, Original Medicare typically covers intermittent skilled nursing, physical therapy, occupational therapy, speech therapy and home health aide visits at 100 percent, with no copay. Our care advisors verify coverage for you before care begins, at no charge.
How do you screen your caregivers and nurses?
Every caregiver and nurse passes a national and Nevada background check, fingerprinting, license and certification verification, reference checks, a TB test and drug screening before they ever enter a home. They are our employees, not contractors, which means they are bonded, insured and covered by workers compensation, and they receive ongoing training and RN supervision.
Will we always see the same caregiver?
Consistency is one of our founding promises. We match each client with a small, dedicated team, usually one primary caregiver plus a familiar backup, and we never send a stranger without introducing them first. If a match does not feel right, we will change it, no questions asked.
Do you require long-term contracts or deposits?
No. Care is week to week, you can change or stop services with 48 hours notice, and there are no deposits, enrollment fees or cancellation penalties. You should stay with us because the care is excellent, not because a contract says you must.
